Mukhya Mantri Lok Sevak Arogya Yojana (MMLSAY)

Mukhya Mantri Lok Sevak Arogya Yojana (MMLSAY) is a medical reimbursement scheme for government employees and pensioners in Assam. It also covers their dependents. 

Features 

• Claims can be submitted online

• Admissible amounts are processed and reimbursed

• Government employees and pensioners who draw salary and pension from the state exchequer can register
• Reimbursement is at CGHS rates
• Employees and pensioners can register themselves or through kiosks

Applying for MMLSAY 

• Go to Sewa Setu

• Log in with your credentials

• Click the Apply Online tab

• Select the scheme you want to apply for

• Fill up the form and upload the required documents

• Pay the requisite amount

• An acknowledgement will be generated with the application reference number

• You will receive an SMS and email alert when the scheme is completed

Guidlines

1. If the applicant is a first time user, he/she needs to click the Register Tab provided in top right corner in this page and then get registered with the necessary fields such as Full Name, Mobile No., Email id (which is not mandatory), State and Password.
2. Then the applicant needs to login with the credentials, by clicking the Login Tab.
3. After the applicant has logged on to the system, he/she needs to click the Apply Online tab and select the scheme they want to apply for and click the apply button after going through the Requirements tab.
4. Applicant has to fill up the form and upload the required supporting documents.
5. On final submission of the form, applicant has to pay the requisite amount.
6. Once the payment is successful it will be forwarded to the higher officials for further processes. An acknowledgement will be generated containing the application reference no., the same will be notified over SMS and Email id of the applicant.
7. An SMS and Email alert is sent to applicant on completion of the scheme. The applicant needs to respond to the SMS and Email regarding completion of the scheme.
8. After successful completion of the scheme, Mukhya Mantri Loksevak Arogya Yojna (MMLSAY) is generated. The applicant will be notified about this over SMS and Email and the applicant can download it from ARTPS portal.
